3 COUNTIES CANCER NETWORK PERSONAL FOLDER
The latest version of the folder is now available online (please see the link below. The folder is available to all patients with a cancer diagnosis. It contains information such as Support Advice – including financial support, prescription charges and travel insurance; Professional Support – including Allied Health Professionals, counselling, lymphoedema services, patient and carer involvement, supportive and palliative care and services offered by hospices and other information useful to patients and carers. It also has a pocket for the Keyworker Card and a plastic pocket for other information to be inserted.
Copies can be obtained from the Clinical Nurse Specialists or from Information Centres. If you would like a folder and are unable to obtain one, please contact Lynne Kitson (contact details from the menu link on the left). A PDF file format is also available for download, chapter by chapter (please see the links below).

Personal Folder - Version 6 (April 2012) - Chapters

Chapter 1 - Introduction. This chapter introduces the folder and tells you about the 3 Counties Cancer Network.
Chapter 2 - General. This chapter lists questions you may like to ask, tells you about cancer registration, has details of transport and car parking, has space to write appointments and medications and has information about blood tests.
Chapter 3 - Contact Details. This chapter has information about your Keyworker and has space for you to write details about your healthcare team and local oncology service. It also has information about the Mobile Chemotherapy Unit, The Hereford Suite in Cheltenham General's Oncology Unit, lists accommodation near to Cheltenham General Hospital and has information about PALS - the Patient Advice and Liaison Service.
Chapter 4 - Tests. This chapter lists and explains a little about some of the tests you may be offered.
Chapter 5 - this chapter is left blank in the folders to insert a cancer and hospital site specific section - please see the paragraph below.
Chapter 6 - Information. This chapter explains what an Information Prescription is, lists information centres and other areas of support within the Network, details documents written by patients for patients within the Network, lists national information resources, signposts carers for information, talks about the emotional impact of cancer and talking to children when an adult has cancer.
Chapter 7 - Support Advice. This chapter gives information about managing fatigue relating to cancer, lists where you can go for benefits advice and contains information about nutrition, prescription charges (free for cancer patients), travel insurance and wigs.
Chapter 8 - Professional Support. This chapter lists the various Allied Health Professionals and the support they give; gives details about Complementary Therapies, lymphoedema services, the Multi-disciplinary Team (MDT), patient and carer involvement, the 3 Counties Cancer Research Network Clinical Trials Team, the role of the Social Worker and living with cancer.
Chapter 9 - Hospice and Day Care Centres. This chapter introduces the hospice and explains the support that they can offer to patients living with cancer. It lists the hospices in the 3 Counties Cancer Network along with contact details. It also lists Day Care Centres, what they offer and their contact details.
Chapter 10 - Folder Comments. This chapter acknowledges the input from patients, carers and healthcare professionals. It also gives details of any welcome feedback about the folder as well as detailing the version history.
